Please follow the below steps to schedule your COVID-19 test:
1. Use the Registration Link or QR Code in the PDF below to access registration for the event
2. Complete the COVID-19 questionnaire, including all consent forms, and time slot selection for each patient that will be tested
3. Bring a copy of your insurance card and state issued ID with you to the event
4. Expect to receive your results within 2 days from the date of testing via text message or phone call from the doctor. If you have not been contacted by us within 2 days, please email us
